Registered Nurse- Position Description:
- The Registered Nurse assumes the responsibility and accountability for the practice of professional nursing in the manner consistent with the Michigan State Practice Act and the American Nurses Association Standards for Home Health Nursing.
Position Requirements:
- Graduate Nurse from an accredited school of nursing preferred.
- Current Michigan State RN license.
- Recent experience with medical-surgical nursing skills.
- Use of an insured automobile for work and a valid Michigan driver's license.
Reports To: Director of Patient Care Services and/or the Administrator. Responsibilities:
- Performs comprehensive subjective admission and ongoing assessment of client status that includes physical, psychosocial and environmental parameters.
- Coordinates delivery of care along health care continuum utilizing community resources, discharge planning and case management.
- Communicates effectively and in accordance with Caring Nurses of Michigan requirements.
- Uses professional nursing judgement to delegate selected nursing tasks when determined safe and appropriate.
- Attendance at and participation in in-service education programs provided by the organization and required by Federal Regulations.

Licensed Practical Nurse
- Position Description:
- The Licensed Practical Nurse (LPN) provides quality care to the patient according to the physician's orders and under the supervision of the Registered Nurse. The LPN supports the patient and family in physical and emotional crisis. He/she functions within the standards published by the Nurse Practice Act of Michigan and the policies of Caring Nurses of Michigan, Inc.
Position Requirements:
- Must be a graduate from an accredited school of practical nursing licensed in the State of Michigan>
- Possesses a minimum of three years of nursing experience. (Community Health, Private Duty, Medical or surgical experience preferred)
- Must currently be licensed to practice practical nursing in the State of Michigan.
- Use of an insured automobile for work and a valid Michigan driver's license.
Reports To: The Registered Nurse in charge of the patient and/or the Director of Patient Care Services. Responsibilities:
- Accepts patient care assignment from the Registered Nurse and possesses the ability to follow the plan of care that has been written by the Registered Nurse.
- Capable of recording and reporting all appropriate matters concerning the patient.
- Attendance at and participation in in-service education programs provided by the organization and required by Federal regulations.

Home Health Aide
- Position Description:
- The Home Health Aide provides physical care and emotional support using basic nursing skills as assigned to the patient. The aide maintain a safe environment in the home and assists patients to regain independent living. The aide will be sensitive to family relationships in as much as good health care is maintained for the patient.
Position Requirements:
- A high school diploma or evidence of completion of the General Education Diploma.
- Proof of completion of an Aid Training course that meets the Federal requirements for condition of participation, August 14, 1989.
- Successful completion of competency evaluation and skills test given by an agency that meets Federal Requirements.
- Use of an insured automobile for work and a valid Michigan driver's license.
Reports To: Director of Patient Care Services, the Administrator and the Registered Nurse who is in charge of the individual patient. Responsibilities:
- Receive work assignments from supervisor or alternate RN.
- Provide quality patient care according to the Standard of Caring Nurses of Michigan, Inc.
- Attendance at and participation in in-service education programs provided by the organization and required by the Federal Regulations.
- Have the ability to work in homes without direct supervision.

Physical Therapy Assistant- Position Description:
- The Physical Therapy Assistant administers non complex physical therapy services to patients according to a written plan of treatment and under supervision of a Registered Physical Therapist.
- Position Requirements:
- Graduation from a two year college level program approved by the American Physical Therapy Association.
- Experience preferred, but not necessary.
- Use of an insured automobile for work and a valid Michigan driver's license.
Reports To: Registered Physical Therapist. Responsibilities:
- Capable of receiving instruction from and supervision by a Registered Physical Therapist.
- Capable of administering the physical therapy program to the patient as established by the Registered Physical Therapist.
- Capable of documenting progress notes of visits and submitting notes to the clinical record at leaston a weekly basis.
- Attendance at and participation in in-service education programs provided by the organization and required by Federal regulation.

Occupational Therapy Assistant- Position Description:
- The Occupational Therapy Assistant administers non complex therapy services to patients according to a written plan of treatment and under supervision of a Registered Occupational Therapist.
Position Requirements:
- Graduation from a two year college level program by the American Occupational Therapy Association.
- Experience preferred, but not necessary.
- Use of an insured automobile for work and a valid Michigan driver's license.
Reports To: Licensed Occupational Therapist. Responsibilities:
