QUALITY ASSURANCE


Caring Nurses of Michigan has developed and implemented key Quality Assurance Criteria for meeting patient needs.


Staffing Requirements:


All staff receive an in-office orientation prior to visiting patients. Clinical
performance of Registered Nurses, Therapists and Home Health Aides are
evaluated by our RN supervisor prior to independent performance with a
patient. The employee's clinical performance is re-evaluated by the
supervisor weekly. After the three month probationary period, the
employee's clinical performance is re-evaluated by the supervisor annually.

Education & Training


To insure quality patient care, our staff receives on-going education and
training, including monthly in-service programs.

Daily Patient Care Review


The Quality assurance Department reviews physician's referrals to insure the
patient meets the criteria for home healthcare services as required by medical
regulation. The QA Department daily monitors patient care services by:


  1. Reviewing all documentation to insure that the treatment plan is being
    followed and implemented by the clinical staff.


  2. Ensuring that there is coordination of services, prompt reporting of
    changes to physicians, the patient's condition is monitored appropriately,
    and that the patient's needs are being met.


  3. Utilizing flow sheets in all the patient's records to monitor changes in the
    patient's condition.


  4. Ensuring that all staff are counseled weekly as needed by our QA
    Department for any problem with patient care or other concerns.


  5. Ensuring that upon patient discharge:

  • Patient and family are knowledeable of disease process,
    medications and treatment plans.


  • Patient has greatest achievable health benefit, with minimal
    unnecessary risk and use of resources.

  • Staff Performance Evaluation:

    Staff Performance is being monitored weekly by conference calls between the Agency's coordinators and the patient/patient caregiver. A Patient care evaluation form is provided for each patient or patient caregiver to receive feedback in order to improve care given.

    A Physician's questionaire form is provided for each physician to verify satisfaction in care provided.

    Clinical Record Review:


    Clinical Record Review is done quarterly by personnel not involved in that patient's care to insure that the plan of treatment is strictly followed, that there is coordination of services, there is prompt reporting of changes to the physician and to insure that the patient's condition was monitored appropriately and the patient's needs were met.
