ELIGIBILITY

To receive services, patients must be:

• Under the regular care of a physician.



• Homebound in a safe environment (you do not need to be bedridden to be homebound).



• Have a need for intermittent (part time) skilled nursing care, physical or speech therapy.



• Additional services of occupational therapy, medical social work and health aide assistance are available once skilled services are established.
